IV Insulin Dosing Proves Safe During Labor with New Study

Recent findings reveal the effectiveness and safety of EndoTool IV for managing insulin during labor and delivery in pregnant patients.

PALO ALTO, Calif.--(BUSINESS WIRE)--October 28, 2026--Glooko, Inc., a leader in digital health solutions, has shared new real-world insights at the Diabetes Technology Society (DTS) meeting regarding the safety of intravenous (IV) insulin dosing during labor and delivery (L&D). These findings highlight the capabilities of the EndoTool IV system for managing glycemic levels in pregnant patients.

Study Overview

The research, titled “Use of a Computerized Intravenous Insulin Dosing Application in Pregnancy,” assessed patient outcomes when using IV insulin during L&D, a critical period requiring optimal blood glucose control. The study emphasized the balance between managing hyperglycemia while preventing hypoglycemia.

Key Findings

In this retrospective analysis, 267 patients were evaluated from 43 different facilities, using data from the 2025 EndoTool IV database. The study compared people with Type 1 diabetes, gestational diabetes (GDM), and Type 2 diabetes to gauge the efficacy of the IV dosing application. Here are some highlighted results:

  • No severe hypoglycemia recorded: There were no cases of severe hypoglycemia (blood glucose levels dropping below 40 mg/dL) in any of the groups studied.
  • Low occurrences of mild hypoglycemia: Instances of blood glucose readings falling below 60 mg/dL were minimal, reported at 0.63% for Type 1 diabetes and 0.16% for Type 2 diabetes and GDM.
  • Manageable hyperglycemia: Hyperglycemia (blood glucose readings above 180 mg/dL) was also low, below 10% of all readings. Only six instances of readings exceeded 300 mg/dL.
  • Optimal glucose management: The majority of patients (80-90%) maintained blood glucose levels in the comfortable range of 90-120 mg/dL.

Importance of Tight Glycemic Control

Joseph Aloi, M.D., Chief of Endocrinology and Metabolism at Advocate Health, Wake Forest University School of Medicine, noted the significance of these results. He explained that labor and delivery can swiftly alter insulin requirements, making meticulous glycemic management essential while mitigating the risk of hypoglycemia. The findings demonstrate that EndoTool IV effectively supports stringent control of blood glucose in pregnant individuals, a critical factor for maternal and fetal health.

Broadening Diabetes Care Continuity

These findings underscore the potential to enhance diabetes care throughout pregnancy. From outpatient monitoring to in-hospital management during labor, seamless transition and continuity of care are vital. Glooko has launched its GestationalCare™ module, a tailored platform allowing healthcare providers to review glucose patterns efficiently, identify patients needing further attention, and ensure consistent management.

Mark Clements, M.D., Ph.D., Glooko's Chief Medical and Strategy Officer, expressed the need for robust monitoring not only during delivery but throughout the entire pregnancy journey. He emphasized the importance of integrating tools like EndoTool IV into a comprehensive approach to diabetes management.

Making a Difference in Diabetes Management

“Effective diabetes management during pregnancy transcends hospital limits,” remarked Dr. Clements. “Patients benefit from consistent outpatient oversight, precise insulin administration at delivery, and continued support after childbirth.” The data presented reflect the potential for EndoTool IV to significantly improve outcomes, underlining Glooko's commitment to a cohesive management strategy throughout pregnancy.

Dr. Clements added that the firm’s efforts in expanding digital tools aim to foster improved connectivity in diabetes management. As clinical needs change swiftly, a system that provides timely data and promotes coordinated decision-making remains crucial.
