The CDC has recorded 19,595 laboratory-confirmed cases of cyclosporiasis between 1 May and 8 September 2026, including 1,043 hospitalisations and two deaths. The agency is simultaneously running eight listeria outbreak investigations and 21 involving salmonella.

What cyclospora is

Cyclospora cayetanensis is a single-celled parasite spread through food or water contaminated with faecal matter. It is not passed directly from person to person: freshly shed oocysts need days to weeks in the environment before they become infectious, which is why household spread is not the pattern and why contaminated produce is.

The illness is a watery diarrhoea that can persist for weeks and tends to relapse -- people improve, assume it is over, and then deteriorate again. Appetite loss, fatigue and weight loss are common. It responds to a specific antibiotic, which matters, because it will not resolve reliably on its own and the drug used is not one that would be given empirically for ordinary gastroenteritis.

Why the case count is what it is

Cyclosporiasis is seasonal in North America, clustering through late spring and summer, and outbreaks have repeatedly been traced to fresh produce eaten raw -- herbs, salad leaves, soft fruit. Confirmed cases also understate the true number by a wide margin, because a specific test has to be requested; a laboratory looking for bacteria will not find this parasite.

The wider picture

Eight active listeria investigations and 21 salmonella investigations is a substantial workload and reflects improved detection as much as anything else. Whole genome sequencing now links cases that would once have looked unconnected, which surfaces outbreaks that previously went unrecognised -- and also produces more recall notices for the same underlying rate of contamination.

Practical steps

Wash fresh produce under running water, including items with skins you do not eat. Washing does not reliably remove cyclospora, which adheres tightly, but it helps with other pathogens and is worth doing. Keep produce away from raw meat during preparation, and refrigerate cut fruit and vegetables promptly.

When to seek care

Diarrhoea lasting more than three days, blood in the stool, fever above 38.9C, signs of dehydration, or symptoms that improve and then return all warrant medical attention. That last pattern is characteristic of cyclospora, and mentioning it -- along with any recent produce you remember eating -- helps get the right test ordered.

Bottom line

A heavy season, and a reminder that persistent diarrhoea deserves a diagnosis rather than a wait.
