The scale of the finding
Reviews of this size are useful precisely because gut symptoms are common in everyone. A modest difference is invisible in a single clinic's caseload and only becomes clear when very large populations are compared. Across the pooled data, the association was consistent in direction and large enough not to be explained by chance.
Several explanations, none yet settled
The obvious candidate is medication: stimulants suppress appetite and change eating patterns, and appetite suppression reliably alters bowel habit. But associations have been reported in people who have never been treated, which argues that medication is part of the picture rather than all of it.
Behaviour is a second route. Irregular meals, low fibre intake, inconsistent hydration, delayed responses to the body's signals, and disrupted sleep all affect the gut, and all are more common where executive function is a daily difficulty. Third, there is genuine interest in shared biology -- the gut and the brain communicate continuously through neural, immune and microbial channels, and some of the neurotransmitter systems implicated in ADHD operate in the gut as well.
Why it is worth clinicians knowing
Gut symptoms in someone with ADHD are easy to attribute to the diagnosis or to the medication and leave there. The practical consequence of this analysis is the opposite: they should be assessed the way they would be in anyone else, because the rates of ordinary treatable conditions are higher, not because a new explanation is needed.
What the study does not show
It does not show that ADHD causes gut disease, or the reverse. Nor does it justify stopping treatment. Stimulant medication has one of the better evidence bases in psychiatry, and the risks of stopping it are concrete while the gut benefit of doing so is hypothetical.
Bottom line
If you or your child have ADHD and persistent digestive symptoms, raise them as their own problem rather than assuming they come with the territory. Changes in bowel habit, blood, weight loss or pain that wakes you at night need assessing whatever else is going on.
